Below is a general guide to South Atlantic’s admissions requirements. To begin the process, we will need:
- Payer source to include traditional Medicare and managed care
- Physician order for a skilled nursing facility
- Current health history and physical
- Current medication list
- Therapy notes or order for therapy
- Discharge summary
- Negative TB skin test given with the last 30 days (documentation provided) with a signs and symptoms form completed (varies per state depending on regulation) or chest X-ray with “no active TB” noted and signed by a physician
